Please note: Your participation in this survey is voluntary.

What Information Do We Collect from You?

We do not collect any personal or personal health information. Although we collect age, gender, weight, drinking preferences, behaviours and history, we do not collect your full name and this information cannot be linked back to you. You are asked to enter your first name so that your report is personalized, but if you prefer not to provide it or to enter only an initial rather than your first name, you may do so.

The data collected on the survey is therefore de-identified and only in aggregate form.

Who Has Access to Your Information?

Relevant Back on Track program staff and Centre for Addiction and Mental Health (CAMH) researchers will have access to the de-identified and aggregated survey information. They will not have access to any personal or personal health information.

How Do We Use Your Information?

The information will be used for research purposes by CAMH and the Ministry of Transportation Ontario (MTO). At CAMH, the survey data will be used to inform how the Back on Track program may be improved in the future.
